Government nurse salary in Malaysia 2026: U5 gross is over RM2,900, not RM1,974
A U5 nurse's base pay is RM1,974 — but with allowances the real gross is over RM2,900. Here's the full breakdown of every allowance, sourced from JPA.
If you look at base pay alone, a U5 nurse starts at RM1,974 a month. But that is not the figure that lands in your account.
With fixed allowances, the real gross for a U5 nurse in the Klang Valley is about RM2,934 a month. Most other articles stop at RM1,974 — and that answers the wrong question. The question you're actually asking is: what do I take home?

The nurse's allowances, one by one
- ITKA — RM160. A fixed allowance for all civil servants, by grade. Grade 5 gets RM160.
- ITP — RM300. Housing allowance. RM300 for grades 1 to 9. (If you live in government quarters, you get only 25%.)
- BISH — RM350. Cost-of-living aid (formerly called COLA). The implementer rate in Zone A is RM350. All of the Klang Valley — KL, Putrajaya, Shah Alam, Klang, PJ, Kajang — is Zone A. Other zones are lower.
- BIPK — RM150. Critical service allowance. Nursing is a critical scheme, so nurses do qualify. (This matters: teachers do not get BIPK because teaching is not a critical scheme — don't trust articles that treat the two the same.)
The effect of the Phase 2 rise (January 2026)
On 1 January 2026, the government raised SSPA pay by 7% for the implementer group. This has already taken effect — it's not coming.
The key thing to understand: the 7% applies to base pay only, not allowances. Allowances stay at their fixed rates. So for a U5 nurse who was already in service on 31 December 2025:
- Base pay: RM1,974 × 1.07 = RM2,112
- Allowances (unchanged): RM960
- Gross after Phase 2: about RM3,072
Salary range by grade
Nurse grades use the U prefix — U5, U7, U9, U10, U12, U13, U14. Under SSPA, the old grade U29 is now U5 and U41 is now U9 (the letter stays, only the number was made smaller).

Remember: the grade minimum (RM1,650 for U5) is not your starting salary. A new nurse at U5 starts at RM1,974 — the starting salary is set higher than the floor of the range.

- Private hospitals don't follow this schedule at all. We have no official private-sector data.
- Shift and duty allowances (night, weekend) vary by hospital and roster — they're not a fixed rate, so we don't include them in the gross calculation above. Your actual figure may be higher.
- Whether new entrants in 2026 start at RM1,974 or RM1,974 plus 7% — JPA hasn't published a Phase 2 starting-salary schedule. The +7% calculation above is for nurses who were already in service before 1 January 2026.

- Do nurses get the critical service allowance (BIPK)?
- Yes. Nursing is a critical scheme, so nurses qualify for BIPK (Critical Service Incentive Payment) of RM150 a month. This is different from teachers, who do not get BIPK because teaching is not a critical scheme.
- What is a nurse's salary after the Phase 2 rise?
- The Phase 2 rise (7%) took effect on 1 January 2026 and is calculated on base pay only, not allowances. For a serving U5 nurse, base pay rises to about RM2,112, making the gross about RM3,072 a month.
- What is U41 called now?
- Under SSPA, the old grade U29 is now U5 and U41 is now U9. The letter 'U' is kept, only the grade number was made smaller.
