Government doctor salary in Malaysia: grades UD9 to UD15 (SSPA)

A medical officer's base pay starts at RM3,130 a month at grade UD9 — the highest starting salary among the commonly-applied civil service schemes. But for a doctor, base pay isn't the full story.

On top of base pay, doctors receive fixed allowances (ITKA, housing, cost-of-living) and the Critical Service Incentive Payment (BIPK) — a critical-scheme payment far larger than in other schemes. It's BIPK that pushes a doctor's gross salary high.

What we can confirm, and what we can't. The base BIPK rate for medical officers — RM750 a month — is in the JPA circular, so we show the full gross breakdown for the UD9 entry grade below. What we can't yet confirm is the BIPK rate for higher and specialist grades: the figures circulating elsewhere contradict each other (from RM750 to over RM2,500) and we found no single official source for them. We also don't publish variable allowances such as on-call payments, because they differ by hospital. Once we have the official rate for specialist grades, we'll add it here.

Medical officer grades

The scheme uses the UD prefix: grades UD9 to UD14, with UD15 for Specialists.

Under the old SSM system, a doctor's entry grade was UD41. After SSPA took effect on 1 December 2024, it is now UD9.

Notice that the UD9 starting salary (RM3,130) is higher than the floor of that grade's range (RM2,250). The starting salary is set in the service scheme, not at the bottom of the range.

(Warning: several other sites quote a UD9 doctor starting salary of around RM5,197. That's wrong — the official SPA page states RM3,130 for UD9 entry. The RM5,197 figure appears to be for a higher grade after promotion.)

What a UD9 doctor's gross salary really is

Doctors are in a critical scheme, so they receive BIPK of RM750 — the highest such allowance among the commonly-applied schemes:

What the schedule doesn't show

This matters, and it's often left out of other articles:

• Allowances aren't in the grade schedule. The table above is base pay only. The fixed allowances (BIPK RM750, ITKA, ITP, BISH) are in the JPA circular and we show them in the UD9 gross breakdown above. What we haven't confirmed from an official source: the BIPK rate for specialist and higher grades, and on-call payments, which vary by hospital. Both can be a large part of a doctor's monthly income, so we don't estimate them.
• The houseman and medical officer training periods have their own conditions that aren't captured in this grade schedule.
• The private sector doesn't follow this schedule. We have no official data for private doctor salaries in Malaysia.
• The Phase 2 rise (7%) took effect on 1 January 2026 — on base pay, not allowances. JPA hasn't published a new schedule; the rise is calculated on each person's individual salary.
